COSTUME REGISTRATION FORM 2011

A MINIMUM DEPOSIT OF $60 is required for costume registration. ALL DEPOSITS ARE NON-REFUNDABLE (no exceptions). The balance of the costume fee is due on the day of pick up (see below for pick up days).

Costume pick up will be from Monday July 23, 2011, (there will be no costume pickup on Thursday July 28th) to Friday July 29, 2011.
